Role Summary

• 13-week assignment in Allegany, New York.
• 8-hour evening shifts: 2:00 PM–10:30 PM.
• Every other weekend required.
• May be asked to work holidays if needed.

Required Qualifications

• Valid LPN / LVN license in New York.
• Long Term Care experience within the last year.
• 1 year of Long Term Care experience.
• BLS certification.

Preferred Qualifications

• IV certification is a bonus.

Clinical Setting

• The assignment includes memory care, skilled care, and assisted living.
• Patient population includes Sisters, Friars, and one Brother.
• Common conditions include CHF, HTN, DM, old strokes, Alzheimer’s, Dementia, and total care needs.

Responsibilities

• Administer tube feedings.
• Check blood sugars.
• Administer insulin, vaccinations, and PO, SQ, and IM medications.
• Perform skin assessments.
• Support care across all levels, including independent-SNF and memory care.
• Help with admissions as needed.

Unit Details

• Staffing ratio is 2 LPNs: 1 on memory care and 1 on skilled, with shared coverage of assisted living.
• Maximum census is 25.
• Day shift goal is 3 LPNs per shift.
• Nurse aides are available.
• Supervisor and assistant supervisor are RNs in the building.

Float / Coverage

• Will be trained on all units and expected to float throughout the building.

Equipment / Clinical Support

• Respiratory equipment includes O2 and CPAP only.
• No trachs or chronic vents.
• Breathing treatments and inhalers are used.
• IV antibiotics are very rare.

Schedule Notes

• Rotating schedule, with flexibility needed for PTO or FMLA coverage.

Dress Code / Conduct

• Solid-color scrubs in any color.
• No facial piercings.
• No offensive tattoos.
• No face or neck tattoos unless covered.
• No smoking on the premises, including the parking lot.
• No earbuds or phone use in patient care areas.
• Breaks only.
