Job Summary

Provide individualized nursing care to residents in a skilled nursing facility, working collaboratively with physicians, nursing staff, residents, families, and other departments. The role requires adherence to facility policies and professional standards to ensure high-quality patient care.

Required Qualifications

• Valid LPN/LVN license in California
• Minimum of 1 year of LPN/LVN experience, with experience within the last year
• Current BLS certification
• Experience working in a skilled nursing facility setting

Specific Nursing Duties

• Conduct individualized patient assessments and notify appropriate staff of abnormal findings.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Perform treatments and procedures as ordered by physicians (e.g., EKGs, blood draws, IV starts).
• Administer medications following facility policies, ensuring accuracy and documentation.
• Provide patient and family education tailored to individual needs.
• Respond promptly and appropriately to emergency situations, including resuscitation measures.
• Maintain patient confidentiality and provide culturally sensitive care respecting patient dignity and rights.
• Report patient conditions during shift changes.
• Adhere to safety and infection control standards.
• Assist other nursing departments as needed and participate in patient transfers and specimen collections.
• Support orientation and education of new staff members.
• Participate in ongoing training, including annual CPR and in-service education.
• Follow medication administration and narcotics control procedures accurately.
• Conduct end-of-shift walking rounds to ensure continuity of care.
• Respond promptly to patient call lights and ensure delegated nursing care is completed and documented.

Professional Requirements

• Adhere to dress code; maintain a neat and professional appearance (Caribbean Blue scrubs required).
• Complete annual education and regulatory requirements.
• Wear identification while on duty.
• Attend annual evaluations and staff meetings.
• Demonstrate commitment to the facility’s mission, ethics, and goals.
• Report to work on time and complete assigned duties within scheduled hours.

Working Conditions

• Willingness to work beyond normal hours and temporarily in other positions as needed.
• Interaction with personnel, visitors, and government agencies when necessary.
