Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 06:45 AM – 07:15 PM
Job Summary
Seeking a Registered Nurse specialized in Operating Room (OR) for a long-term assignment.
Required Qualifications
- Valid RN license in Idaho
- Minimum of 1 year of Operating Room (OR) experience, with OR experience within the last year
- BLS certification issued by the American Heart Association
- ACLS certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Onboarding and Compliance Requirements
- Current and valid license/certification verification dated within 30 days of start.
- Fit test for respiratory protection.
- Negative 10-panel drug screen completed within 30 days of start (no THC).
- Signed Code of Conduct and Confidentiality & Security Agreements.
- Specialty-specific competency testing completed within the last year.
- COVID-19 vaccination proof including booster or approved exemption/declination (valid through October 2026).
- Proof of MMR vaccination or immunity.
- Hepatitis B vaccination series initiated prior to start.
- Influenza vaccine is optional.
- TB screening: 2-step PPD or IGRA with chest x-ray if positive (per regional requirements).
- Proof of Varicella vaccination or immunity.
- Physical/health statement confirming fitness to work within the last year.
- Criminal background check (federal, nationwide, county) within last 7 years, dated within 30 days of start.
- Education verification of highest academic degree or diploma.
- EPLS and OIG exclusion checks dated within 30 days of start.
- Integrity and audit services form signed shortly before start.
- Joint Commission CORE exam within the last year.
- National Sex Offender Registry (NSO) check dated within 30 days of start.
- System access form completed and uploaded one week before start.
- Skills checklist provided and completed for clinical contractors.
Additional Notes
- All certifications requiring images must include front and back of the card.
- All documentation must be uploaded to the credentialing system prior to or at onboarding.
- Declinations for vaccines are not accepted in California or Illinois (not applicable here but noted).
