Overview

• Role: Hospital Acute Care OT
• Employment type: Travel/Contract
• Location: Pleasant Prairie, Wisconsin

• Schedule: Monday-Friday, 5 eight-hour shifts
• Shift: Day (5x8 Hours) 7:30 16:00
• Contract length: 13 weeks

Required Qualifications

• Valid Occupational Therapist license in Wisconsin
• Occupational Therapist (1 year of experience)
• Occupational Therapist experience within the last year
• Background check required within 1 year of start, including SS Trace, county searches for the last 7 years of locations lived/worked/in school, nationwide search, and sex offender search
• Wisconsin - Caregiver Search Results required within 90 days of start
• 2 professional references verified within 2 years of start
• Photo for badge
• Confidentiality statement signed prior to start
• Physical exam within 1 year of start
• Negative TB or chest X-ray within 1 year of start
• Drug testing within 14 days of start
• Flu vaccine or medical/religious exemption required during flu season
• Skills checklist required annually
• Competency exam required as applicable to profession/specialty
