• Guaranteed hours: 36 hours per week
• Service area: Santa Clara, Sunnyvale, Cupertino, Milpitas, Mt. View, Campbell, Los Altos, Saratoga, Los Gatos, and San Jose

Responsibilities

• Home health visits with a typical caseload of 5–6 patients per day
• May visit up to 6 patients per day
• Expected to maintain timely communication and documentation
• Weekend visits may be picked up to help meet productivity expectations
• Must have personal transportation
• Occasional travel radius may exceed 50 miles

Required Qualifications

• Valid Occupational Therapist license in California
• Occupational Therapist (1 year of experience)
• Occupational Therapist experience within the last year
• One completed travel assignment
• BLS - Basic Life Support issued by American Heart Association or American Red Cross
• Experience in a Home Health setting

• First-time travelers are not accepted
• Local candidates are accepted
• On-call is not required
• Documentation tools are provided and must be returned
• Any color scrubs are allowed
• Free parking is available
• Mileage reimbursement is provided at the IRS standard rate
