Speech-Language Pathologist (SLP) - Home Health (Travel/Contract)
Location: Walla Walla, Washington
Contract Length: 13 weeks
Position Details
- Specialty: Home Health Therapy/Rehabilitation
- Shift: 4 hours Monday-Friday OR two 8-hour days plus one 4-hour day
- Guaranteed Hours: 20 hours per week
- Types of Cases: Geriatric patients
- Territory: Up to a 60-mile radius from the office/hub
- Dress Code: Scrubs (any color), no jeans
- Device Provided: iPad
- Mileage Reimbursement: Yes, at IRS standard rate
- Cell Phone: Not allowed for work use
Required Qualifications
- Valid Speech-Language Pathologist license in Washington
- Minimum 1 year of recent experience as a Speech-Language Pathologist
- Master's degree in Speech-Language Pathology or equivalent education/experience
- Basic Life Support (BLS) certification
- Experience with Homecare Homebase (HCHB)
- Experience with OASIS
Compliance and Background Requirements
- Background checks including SSN trace, county searches (last 7 years), FACIS L1M, nationwide sex offender, motor vehicle records, education and work history verification
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
- Verification of highest degree related to profession
- Signed acknowledgment of Orientation Safety Manual (updated annually)
- Agency background attestation required prior to start
Health and Safety Requirements
- Physical exam within the last 12 months (not required annually)
- Fit test for N95 mask (no specific model required)
- Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if previously positive
- Annual TB questionnaire required
- Immunizations or proof of immunity for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within 12 months of start (instant acceptable)
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID vaccination or facility-specific declination required
Documentation and History
- Two professional references within 2 years of start (travel, allied, per diem acute care)
- Employment application with full 7 years of work history, no gaps
- Annual skills checklist
- Work history verification for 7 years (with attempts to verify if needed)
- Proof of current auto insurance required due to driving responsibilities
Additional Notes
- Locals are accepted and paid at travel rates
- No on-call responsibilities
- Compliance liaison will manage background attestation and onboarding paperwork
