Home Health Registered Nurse

Overview

• Location: Sonora, California
• Employment type: Travel/Contract
• Shift: Day shift, 5x8 hours, 7:00 AM–3:00 PM
• Contract length: 13 weeks

Role Summary

• Plans, organizes, and directs home care services for patients in their homes.
• Manages a patient caseload independently and collaborates with other disciplines to meet care goals.
• Serves as a resource to patients, family members, and the care team.

Required Qualifications

• Valid RN license in California
• Home Health (1 year of experience)
• Home Health experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License

Preferred Qualifications

• Bachelor’s degree in Nursing (BSN)
• Acute care facility or home care experience

Additional Requirements

• On-call duties may be assigned based on location and operational needs.
• All required vaccinations must be completed as a condition of employment, unless a medical or religious exemption applies.
• A 7-year work history with locations and gaps included is required in the application/resume.
• Two clinical references verified within one year of start are required.
• A unit-specific skills checklist is required.
• A physical exam is required within 90 days of start.
• TB screening is required within 30 days of start and must be updated annually.
• Drug testing is required within 30 days before start.
• A current photo for badge use is required.
• A signed contractor compliance education form is required before the hard document due date.
