Registered Nurse – Med/Surg / Telemetry (Travel/Contract)

Location: Lebanon, Indiana
Contract Length: 13 weeks

Job Details

• Specialty: Med/Surg with Telemetry experience required
• Shift: Night shift, 3 shifts per week, 12 hours each (6:00 PM – 6:30 AM)
• Hours: 36 hours per week guaranteed
• Weekend Requirement: Minimum 3 weekends in a 6-week period
• Call-In Policy: If a shift is called in, the nurse must pick up another shift to compensate
• On-Call: Possibility of being placed on call
• Floating: May be required to float to other units depending on hospital needs
• Patient Ratio: 1 nurse to 5 patients
• Experience Required: Minimum 1 year in specialty (Med/Surg with Telemetry experience)
• First-Time Travelers: Accepted
• Local Candidates: Accepted if within 50 miles radius

Required Certifications

• ACLS (AHA only; instructor and provider cards accepted)
• BLS (AHA only; instructor and provider cards accepted)
• PALS (AHA only; required for Med/Surg due to pediatric admissions)
• NIHSS

Required Licenses

• Valid Registered Nurse (RN) license for Indiana

Special Requirements

• Telemetry experience is required
• Ambulatory and Med/Surg experience preferred

• Dress Code: Professional appearance; scrub color is flexible

Background and Compliance Requirements

• 7-year background check including SS trace, county, nationwide, sex offender, and international criminal (if applicable), updated annually
• EPLS/GSA/SAM and HHS/OIG checks annually
• Transcript/Diploma verification required
• Physical exam within 12 months, updated annually
• Fit mask test within 12 months, updated annually (3M 1860R or 1860S masks accepted; no PAPRs; facial hair restrictions apply)
• Negative TB test or chest X-ray within 1 year, renewed annually
• Immunizations or titers required:

  • Tdap (within 10 years or declination)
  • Rubeola (Measles), Mumps, Rubella (positive titer or 2 vaccines with boosters if needed)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (titer, 2 vaccines, history of chickenpox, or declination/booster as applicable)
• Negative 10-panel drug screen within 12 months
• Seasonal flu vaccination or approved medical/religious exemption required annually
• 2 professional references required
• Annual skills checklist and competency exams (including medication/pharmacology and color blindness testing)

Requirements/Qualifications

• Valid Registered Nurse (RN) license in Indiana
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 1 year of Med/Surg and Telemetry experience, with experience within the last year
• ACLS certification
• BLS certification issued by the American Heart Association
• PALS certification
• NIHSS certification

Additional Notes

• No more than 5 days of requested time off (RTO) allowed during assignment
• Professionalism in appearance expected
• Candidates must have PALS certification before start date due to pediatric patients on unit
