RN Case Manager Travel
Location: Fairfield, California
Employment Type: Clinical Nursing Travel
Contract Length: 8 weeks
Job Summary
The Inpatient Case Manager assesses, plans, implements, monitors, and evaluates individualized patient care plans across the continuum of care. This role involves collaboration with team members and Social Services to prioritize daily activities, using clinical knowledge, critical thinking, and strong communication skills to ensure optimal patient outcomes. The position requires independent judgment and ongoing patient assessment, with a focus on financial and health plan resource considerations.
Work Environment & Schedule
- Assigned to various acute care units such as Medical-Surgical, Medical-Telemetry, Surgical, ICU, and possibly Emergency Department (ED)
- 5 shifts per week, 8 hours each (typically 8:00 AM – 4:30 PM)
- Schedule may include a mix of weekdays and weekends based on department needs
- Physical demands include walking, stooping, bending, twisting, sitting, standing, and computer use throughout the day
Required Qualifications
- Valid RN license in California
- Bachelor of Science in Nursing (BSN)
- Minimum 2 years of Case Management experience, including experience within the last year
- Certified Case Manager (CCM) certification
- Acute Care experience
Skills & Knowledge
- Proficient in personal computer use, word processing, and data entry/retrieval from clinical systems
- Knowledge of MCG, MIDAS+, Medicare rules and regulations, and Conditions of Participation
- Familiarity with Joint Commission standards, Department of Human Services, Patient Safety, and Department of Health organizational standards
- Understanding of acute care, home care, subacute care, long-term care, hospice, rehabilitation, community resources, medical necessity guidelines, and quality assurance processes
- Strong critical thinking, clinical judgment, assessment, organization, time management, and problem-solving skills
- Effective communication and collaboration with patients, families, physicians, healthcare personnel, and supervisory staff
- Ability to prioritize and manage assignments efficiently with minimal oversight
- Demonstrates professionalism, team spirit, and a commitment to continuous learning

- Ability to perform essential job functions with reasonable accommodations if needed
- May involve lifting up to 35-40 pounds
