Job Opportunity: Home Health Registered Nurse

Location: Rhinelander, Wisconsin
Employment Type: Travel/Contract
Contract Length: 13 weeks

Schedule

• Monday to Friday, 8:00 AM – 5:00 PM
• No call, no weekends, no overtime
• 40 hours per week (5x8 hour shifts)

Required Experience

• Minimum 12 months of Home Health nursing experience within the past 3 years
• Experience with Home Care Home Base (HCHB) electronic health record system
• Experience with OASIS documentation

Required Qualifications

• Valid Registered Nurse (RN) license in Wisconsin
• Minimum of 1 year of Home Health nursing experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience in a Home Health setting
• Experience with Homecare Homebase (HCHB) electronic health record system
• Experience with OASIS documentation

Background and Compliance Requirements

• Comprehensive background checks including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1M
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records (if driving is required)
  • Education and work history verification
• State-specific background checks as applicable (e.g., Kentucky, Virginia, Indiana, New Hampshire, South Carolina)
• EPLS/GSA/SAM and HHS/OIG checks within 90 days of start
• Fingerprinting requirements vary by state; must be current and cleared
• Employment application with full 7 years of work history, no gaps
• Verification of 7 years of employment history with attempts to verify if needed
• Two professional references within 2 years of start (travel, allied, per diem acute care)

Health and Immunization Requirements

• Physical exam within the last 12 months (physician statement)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
• Annual TB questionnaire required
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (series, titer, or declination)
  • Varicella (series, titer, or declination)
• Seasonal flu vaccination or facility-specific declination form during flu season (Aug-Nov)
• COVID vaccination or facility-specific declination
• 9-panel drug screen within 12 months of start

Facility-Specific Requirements

• Proof of current auto insurance required (for positions involving driving)
• Completion of Acceptable Use Policy prior to start
• Facility-specific flu and COVID declination forms if applicable
