LPN Case Manager

Overview

• Travel/contract role
• Full-time, 40 hours per week
• Day shift: Monday–Friday, 8:00 a.m.–4:30 p.m.
• Home base is Riedman Administrative Campus
• Mobile position with travel during the workday, including potential home visits, hospital visits, and patient office visits
• Adult/Geriatric focused
• Local candidates accepted at a lower rate if under 75 miles
• Accepts first-time travelers

Required Qualifications

• Valid LPN / LVN license in New York
• 1 year of Case Management experience
• Case Management experience within the last year
• BLS
• Experience with Epic

Preferred Qualifications

• Clinical knowledge or experience with CHF, COPD, and multiple complex comorbidities
• Community resource knowledge

Role Details

• Supports the Transitions of Care program focused on reducing 30-day readmission rates
• Works under the direction of the RN Care Manager to help implement the care plan
• Not a hands-on nursing role
• Responsibilities include appointment scheduling, biometric data monitoring, providing and reinforcing education, attending patient visits as needed, completing home visits, connecting patients with community resources, maintaining frequent patient contact, care team rounds, and other tasks directed by the RN
• Manages a monthly rolling caseload of approximately 25–30 patients
• Works with a multidisciplinary care team including RN, Social Worker, Pharmacist, and other professionals
• Documentation is done in the EHR
• Access to reliable transportation is required
• LPNs will not transport patients

Float / Work Locations

• Floating may include Rochester General Hospital, Unity Hospital, patient homes, and provider offices

Contract Length

• 13 weeks
