Hospice Admission Visit RN

• Location: Sunnyvale, California
• Employment type: Contract
• Contract length: 13 weeks
• Schedule: Days; Sunday–Thursday or Tuesday–Saturday, 10am–7pm or 11am–8pm
• Hours per week: 40
• Territory: San Francisco County, San Mateo County, and Santa Clara County

Required Qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Hospice (1 year of experience)
• Hospice experience within the last year
• BLS

Role Summary

• Admits patients to hospice service
• Assesses physical, psychosocial, environmental, safety, and developmental needs
• Develops individualized plans of care
• Documents observations and interventions related to the patient’s environment
• Coordinates care with a multidisciplinary team
• Participates in quality assessment and performance improvement activities
• Conducts follow-up visits as needed
