Job Description
Seeking a Registered Nurse (RN) with Medical/Surgical specialty for a long-term assignment in Nashville, TN.
Required Credentials & Onboarding Requirements
Licenses & Certifications
- Nursys Verification: Must be completed within 30 days prior to start.
- Current Nursing License: Primary source verification within 30 days prior to start.
- BLS (AHA): Must be current and valid.
- NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
- Ascension Drivers License: Required for RN/LPN/PTs working on a compact license; address must match home state for compact license.
- Driver’s License/State ID: Must be current, not expired, and match address on file.
- Ascension Profile: Resume or professional profile required.
- Two Professional References: At least two qualifying references prior to first assignment.
Testing & Exams
- Ascension Medical Surgical Exam: Minimum 80% score, only 2 attempts allowed; must be completed prior to start and updated every 3 years.
- Ascension EKG Exam: Minimum 80% score, only 2 attempts allowed; updated to latest edition as of Dec 1, 2023.
- Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, updated yearly.
- Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
- Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
Health & Immunizations
- MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted.
- Varicella: Proof of two vaccines, serological immunity, or verified history; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Influenza Vaccine: Required for flu season (Oct 1 - Mar 31); exemptions accepted but mask required.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd within 30 days of start; or negative IGRA within 90 days. TB questionnaire updated annually.
- Positive PPD: Negative chest X-ray within 1 year and annual TB questionnaire required.
- Fit Test: Required for all travelers; preferred prior to start, can be completed onsite.
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start and updated annually; includes 12 specified drugs.
Background & Verification
- Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or if break in service > 90 days.
- EPLS Check: Run within 90 days of start; rerun if break in service > 90 days.
- Education Verification: Relevant education verified within 90 days of start; rerun if break in service > 90 days.
- Employment Verification: Past 7 years verified; updated annually or if break in service > 90 days.
- Ascension Confirmation of Assignment: Provided upon initial assignment and with each extension.
Onboarding & Other Requirements
- Ascension Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Onboarding Requirements Education Attestation: Completion of pre-hire 6-hour module with attestation.
- Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
- Ascension OFAC: Required prior to onboarding.
- Ascension Color Vision Test: Completed prior to start and updated every 3 years.
- Ascension Name Change Document: Optional, if applicable.
- Ascension Unit Specific Checklist: Optional, uploaded by unit leader within a week of start.
- Hiring Manager Evaluation: Optional post-assignment evaluation.
