Job Opportunity: Case Manager
Location: San Francisco, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Responsible for chart audits and coordinating meetings/reviews.
- Floating between multiple facilities as needed.
- Shift: Day shift, 5 days a week, 8-hour shifts (8:00 AM - 4:30 PM), no weekends.
Required Qualifications
- Valid RN license in California.
- Minimum of 1 year of case management experience, with experience within the last year.
- BLS certification issued by the American Heart Association or American Red Cross.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Background and Compliance Requirements
- 7-year background check required within 6 months of start, including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, and international search if applicable.
- FACIS Level 3 screening required within 2 weeks of start and monthly thereafter.
- EPLS/GSA/SAM and HHS/OIG checks required and updated annually.
- Official transcripts/diplomas or attestation of verification attempts required for education verification.
- Sex offender search must be run within 90 days of start.
- Drug testing and immunization documentation must comply with state and facility policies.
- Employment application must include full 7 years of work history with no gaps; employment verification required for up to 7 years.
- Two professional references required within 3 years for clinical positions.

- Candidate must review and sign facility-specific documents prior to start, including:
- Job description acknowledgment
- Standards of business conduct
- Code of conduct
- Vendor training (2024 and 2025 versions as applicable)
- Overpayment refund policy
- Regulatory orientation for contracted workers
- Conditional offer of employment attestation must be completed prior to start.
- Religious exemption forms required if applicable for PAPR use or vaccine exemptions.
- Unit-specific skills checklist required and must be renewed annually.
- ICU RNs may be required to take an onsite EKG exam after start.

- Dysrhythmia/EKG competency exam may be required for telemetry units.
