Case Manager (Travel/Contract)
Location: San Leandro, California
Contract Length: 13 weeks
Job Summary
- Travel/contract position covering day shift (8:30 AM – 5:00 PM), 5 days a week, 40 hours/week
- No float or on-call responsibilities
- Weekend work every other weekend
- Accepts first-time travelers
- One return-to-office request allowed
Required Qualifications
- Valid RN license in California
- Bachelor of Science in Nursing
- Minimum 1 year of case management experience within the last year
- Experience in an outpatient setting
- Basic Life Support (BLS) certification issued by the American Heart Association
- Knowledge and experience with admission criteria, care coordination, discharge planning, disease management, and plan of care
- Familiarity with CMS and OSHA standards
Health and Compliance Requirements
- Background checks including nationwide and county criminal searches, OIG, EPLS/SAM, and others within 90 days of start
- Education verification with official documentation (no placeholders)
- Physical exam within 90 days of start, signed by MD, DO, PA, NP, or APRN confirming good health and ability to perform duties
- Fit mask test within 90 days of start (specific mask models required)
- Immunizations and titers:
- Tetanus/Diphtheria/Tdap (within 10 years or signed declination)
- Negative TB test or chest X-ray updated annually
- Measles, Mumps, Rubella (proof of 2 MMR vaccinations or positive IgG titers)
- Hepatitis B (positive titer or completed series/declination)
- Varicella (proof of 2 vaccinations or positive IgG titer)
- Seasonal flu vaccination or signed declination required annually
- Drug screening via oral saliva test within 30 days of start (rapid tests not accepted)
- Color blind exam within 90 days of start (Ishihara test accepted)
Documentation and Other Requirements
- Valid photo ID (Driver’s License or Passport) with clear, color photo
- Current resume including last 7 years of employment
- Employment verification for past 3 years
- References per agency requirements (not facility requirement)
- Completion of orientation safety manual acknowledgment
- Completion of facility-specific competencies and new employee orientation modules on first day
- Badge photo (passport style, no filters) and badge photo request form required
Additional Notes
- Prior travel experience is not required
- This role involves care coordination and case management duties in an outpatient setting with some acute care preferred experience
- Compliance with all facility and agency credentialing and onboarding requirements is mandatory for assignment continuation
