Job Details
- Location: Hannibal, Missouri
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Shift: Day shift, Monday-Friday, 8:00 AM - 5:00 PM (40 hours/week)
- Weekend Rotation: Saturday rotation required; if worked, a weekday off is given to maintain 40 hours/week
- Setting: Inpatient rehabilitation/acute care
- Open to local candidates (no radius restrictions)
Required Qualifications
- Valid Occupational Therapist license in Missouri
Compliance and Documentation Requirements
- Unexpired government-issued photo ID (e.g., driver’s license or state ID)
- E-Verify compliance
- Background checks within 45 days of start, including:
- Social Security trace and county searches for last 7 years
- Nationwide and sex offender registry checks
- Education verification
- EPLS/GSA/SAM and HHS/OIG checks prior to start
- Education verification of highest level achieved
- National certification verification prior to start and upon expiration (as applicable)
- Primary source verification of Missouri OT license prior to start and upon expiration
- Physical exam within 12 months of start
- Immunizations and health screenings:
- Tdap vaccine (updated every 10 years; declinations accepted)
- Negative TB test (PPD, Q Gold, or T-Spot) within 1 year or chest X-ray within 5 years if positive PPD
- Annual TB questionnaire if applicable
- Measles, Mumps, Rubella: 2 vaccines or positive titer (exempt if born before 1957)
- Hepatitis B: series, titer, or declination accepted
- Varicella: 2 vaccines, titer, or history of chickenpox
- Seasonal flu vaccination or declination (mask required if declining during flu season)
- COVID-19 vaccination card or approved exemption letter
- Drug screening: 9-panel drug test within 45 days of start (instant acceptable) covering amphetamines, benzodiazepines, cocaine, marijuana metabolite, opiates, oxycodone, phencyclidine
- Signed acknowledgment of orientation safety manual and Joint Commission training (updated annually)
- Skills checklist and work history verification for past 7 years
- Two professional references within 3 years of hire
- Missouri-specific electronic driver’s license search results required

Nurse:patient ratios are great. This SDU has the best UTL and Director I’ve worked with in quite awhile.
