Licensed Practical Nurse (LPN) - Full Time Evening

Location: Montour Falls, New York
Employment Type: Long-Term Assignment (13 weeks)
Shift: 3:00 PM – 11:30 PM

Job Description

• Assist with assessment, planning, implementation, and evaluation of patient care during shift.
• Provide skilled nursing care meeting established standards.
• Collaborate with interdisciplinary care team and advocate for patients.
• Administer medications and treatments under RN supervision per facility policy.
• Take and report vital signs (respiration, temperature, blood pressure, heart rate).
• Maintain clinical competence and complete required education within specified timeframes.
• Document thoroughly and accurately in EMR/Point Click Care (experience preferred).
• Communicate patient condition changes to nursing personnel and responsible RN.
• Work every other weekend and every other holiday.
• Floating to other units and potential temporary drop to CNA role as staffing needs dictate.

Required Qualifications

• Valid LPN / LVN license in New York.
• LPN / LVN (1 year of experience).
• LPN / LVN experience within the last year.
• BLS issued by American Heart Association or American Red Cross.
• Driver's License.
• Skilled Nursing experience.
• Proof of COVID-19 vaccination or agency declination.
• MMR immunity: Positive IgG titer or two vaccine series required; if negative/equivocal, vaccination documentation required.
• Hepatitis B immunity: Positive antibody titer, 3 vaccine series, or specific declination form required.
• Varicella immunity: Positive IgG titer or two vaccine series required; vaccination documentation required if titer negative/equivocal.
• Tdap vaccination within last 10 years or specific declination form.
• Tuberculosis testing: Must have one of the following within required timeframes:

  • Two skin PPDs (one within 12 months, one within 3 months), or
  • One Quantiferon/Spot test within 12 months.
  • For past positives, chest x-ray within 12 months may be required.
• Annual fit test for approved respirators (specific mask models listed; fit test provided on-site if needed).
• Physical exam within last 12 months with no restrictions, including height, weight, vitals, and vision screening.
• Valid, unexpired U.S. driver’s license or state ID.
• Negative 9+ panel drug screen excluding marijuana (not permitted to be tested in New York).
• Background check including county and national criminal searches, SSN address trace, FACIS 3 check, and others as required.
• Submission of professional references: one supervisor reference from last 12 months and one supervisor or peer reference from last 3 years.
• Completion of required onboarding documents including signed job description, code of conduct, sexual harassment acknowledgement, and others.
• Current license and state license verification completed within 30 days prior to start.
• Skills checklist and core competency exam for the assigned unit, updated annually.
• Resume detailing all employment and education history for the last 7 years.
• Color photo headshot (professional, no filters, JPEG format).

Additional Information

• Floating to other units and potential temporary role adjustment to CNA may be required based on staffing.
• Every other weekend and holiday work is mandatory.
• Experience with Point Click Care EMR system preferred.
• Prior travel experience is not explicitly required.
• Contract length interpreted as 13 weeks.
