Job Description

  • Assess, plan, organize, and participate in rehabilitative programs to help build or restore vocational, homemaking, daily living skills, and general independence for persons with disabilities or developmental delays.
  • Gain confidence and cooperation from patients, families/support groups, and healthcare providers through competent assessment, monitoring, care, and communication.
  • Adhere to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.
  • Provide clinical supervision of assistants and aides.
  • Serve as clinical instructor for assigned students.

Required Certifications

  • State License (primary source online verification required within 30 days of start)
  • BLS (Basic Life Support) - AHA or ARC only
  • ACLS (Advanced Cardiac Life Support) - AHA or ARC only (verify by position)
  • PALS (Pediatric Advanced Life Support) - AHA or ARC only (verify by position)
  • NRP (Neonatal Resuscitation Procedure) - AHA/AAP only, must be Instructor or Advanced Provider Course (verify by position)
  • CCRN (Critical Care RN) - verify by position
  • TNCC (Trauma Nursing Core Course) - valid for 4 years (verify by position)
  • EFM (Electronic Fetal Monitoring) - typically required for Labor & Delivery (verify by position)
  • Tech Certification (National) - required within 30 days of start as applicable
  • IV Certification (LVN/LPN/Other) - may be required for some tech roles (verify by position)
  • Management of Assaultive Behavior (MAB) - verify by position
  • NIH Stroke Certification - verify by position; expiration 1-2 years from date achieved
  • CPI (Crisis Prevention Institute) - verify by position
  • Other certifications may be required depending on specific role and unit

Background Check Requirements

  • 7-year background check within 6 months of start, including:
    • Social Security Trace with all aliases
    • County searches for last 7 years of residence/work/school
    • Nationwide Sex Offender Search (within 90 days of start)
    • Education Verification (not required if highest education is high school)
    • FACIS Level 3 search within 2 weeks of start and monthly thereafter
    • EPLS/GSA/SAM, HHS/OIG, OFAC Sanctions List (if FACIS III cannot be run)
    • Motor Vehicle Report (required for all candidates at this facility)
  • Background checks do not require annual updates by client but Aya Healthcare requires annual background checks for direct candidates.

Health and Immunization Requirements

  • Physical Exam within 1 year of start (must state "cleared for work" or equivalent)
  • Fit Mask Test within 1 year, renewed annually (exceptions apply)
  • Tetanus/Diphtheria/Tdap within 10 years (TDAP required, TD alone not accepted)
  • Negative TB test or chest X-ray within 90 days of start, renewed annually
  • Immunity or vaccination documentation for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (series or declination)
    • Varicella
  • Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per CA law)
  • Seasonal Flu Vaccination or signed declination during flu season (Nov 1 - Apr 30)
  • Color Blind Exam (in-person, if applicable)
  • Respiratory Safety Clearance (not required for some roles)

Competency Exams

  • Medication Test/Pharmacology Competency Exam (as applicable)
  • Competency Exam with minimum passing score of 80% or Angoff score (as applicable)
  • Dysrhythmia/EKG Competency Exam (may be required for some units)

Facility Specific Documents and Requirements

  • Personal or Auto Insurance (required for all candidates at this facility; must be effective for assignment duration)

Employment History and References

  • Full 7 years of work history required in application with no gaps; must include city and state of employment locations
  • Employment verification for up to 7 years required; gaps over 30 days must be explained
  • 2 professional references required for clinical, allied, per diem acute care candidates (within 3 years)
  • 1 professional reference required for per diem non-acute care candidates

  • Religious exemptions for vaccines and facial hair (PAPR) are considered on a case-by-case basis with required documentation.
