May be assigned to either Acute Care or IRC based on volume

Schedule

Mon-Fri
Tuesday-Saturday
Day shift, 5x8 hours
8:00–16:30

Required Qualifications

Valid Physical Therapy Assistant license in North Carolina
Physical Therapy Assistant (1 year of experience)
Physical Therapy Assistant experience within the last year

Assignment Details

ASAP start date
13-week contract

Required Credentials and Compliance

Government-issued photo ID
Background check within 1 year of start covering all locations lived/worked/in school in the past 7 years, including Social Security Trace, counties, nationwide, national sex offender, and international searches if applicable
EPLS/GSA/SAM within 30 days of start and updated for each renewal/extension
HHS/OIG within 30 days of start and updated for each renewal/extension
State License 1 Verification with primary source verification
NURSYS verification, as applicable
NPI number verified before start
Color blind testing
Competency exam, as applicable to profession/specialty
Physical exam within 1 year of start
Tetanus/Diphtheria/Tdap within 10 years of start
Negative TB or chest X-ray, with annual update
TB questionnaire if history of past positive TB
Rubeola, Mumps, Rubella, and Varicella immunity or vaccination documentation
Hepatitis B series, titer, or declination
10-panel drug screen within 30 days of start
Flu vaccine or approved medical/religious exemption during flu season
Two clinical references, with one from a supervisor or higher
Complete application/resume
Skills checklist, updated annually

Notes

Modules are required after contract signing and must be completed by the stated deadline
Photo identification must be carried on person during hours of service to client
