OT Inpatient Rehab - Travel/Contract

Location: San Francisco, California
Shift: Day (5x8 hours), 8:00 AM - 4:30 PM
Contract Length: 16 weeks


Job Description

  • Work in an inpatient rehabilitation setting.
  • Ability to float to other facilities within the system.

Required Qualifications

  • Valid Occupational Therapist license in California.
  • Minimum 1 year of Occupational Therapist experience, including experience within the last year.
  • BLS certification issued by the American Heart Association.

Required Documentation & Compliance

  • Current and valid driver’s license, government photo ID, or US Passport matching professional license.
  • Background checks including 7-year criminal history, EPLS/GSA/SAM, HHS/OIG, sex offender search, and education verification.
  • Physical exam within 1 year of start, including physician statement of fitness to work.
  • Drug testing: Standard 9-panel (excluding THC) within 30 days of start, renewed every 2 years.
  • Immunizations and titers:
    • Tetanus/Diphtheria/Tdap vaccine within 10 years (no declinations accepted except medical exemptions).
    • Two negative TB tests (PPD, QuantiFERON, or Tspot) with specific timing requirements.
    • Measles (Rubeola), Mumps, Rubella: IGG titer or 2 vaccines required.
    • Hepatitis B: Titer, series, or declination (declination must be signed within 1 year).
    • Varicella: IGG titer or 2 vaccines required (history of disease not accepted).
    • Seasonal flu vaccine or facility-specific declination during flu season (Nov-Apr).
  • Fit mask test within 1 year of start (specific approved masks required).
  • COVID-19 vaccination is strongly recommended but not required as of current policy.

Certifications & Competencies

  • Pass/Fail Telemetry and Medication exams on first day of orientation (specific to nursing but may be relevant for clinical staff).
  • Annual competency exams and core training modules with passing scores of 80% or higher.
  • Advanced Cardiac Life Support (ACLS) may be required depending on unit (verify unit-specific requirements).
  • Other certifications (e.g., ACLS, PALS, NRP, CHEMO, TNCC) are unit-dependent and may not apply to OT.

Facility-Specific Requirements

  • Photo for badge must be submitted by the Monday prior to start, with specific photo guidelines.
  • Uniforms: Scrubs provided for some units; others must follow facility scrub color guidelines. Branded scrubs are optional and not reimbursed.
  • Parking and uniform information must be reviewed and acknowledged prior to start.
  • Completion of facility-specific learning modules is required; RNs must complete all modules before Day 1 if starting on a Tuesday.
  • Annual TB questionnaire required within 30 days of start to determine if renewed TB testing is necessary.
  • Cyber Security Awareness Training and HIPAA training required prior to or shortly after start.
  • Confidentiality and acceptable use agreements must be reviewed and signed.

Employment History & References

  • Resume with last 7 years of work history; gaps of 30 days or more must be explained.
  • Verification of 7 years of work history required (third-party verification if needed).
  • Two supervisory references required from most recent or second most recent employer.

Additional Notes

  • Background checks and license verifications must be current within 30 days of start.
  • Some health and compliance documents have specific expiration and renewal timelines; candidates must maintain up-to-date documentation.
  • Candidates must complete COVID-19 related questionnaires and attestations as required by the facility.
  • Candidates may be required to complete respirator medical evaluation and fit testing if applicable.
