Required Qualifications
- Valid Physical Therapist license in North Carolina
- Physical Therapist experience within the last year
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
Additional Requirements
- Government-issued photo ID
- 2 professional references within 3 years of start
- Background check
- EPLS/GSA/SAM screening
- HHS/OIG screening
- Physical exam within 1 year of start
- Negative TB test or chest X-ray within 1 year of start
- Negative 9-panel drug screen within 1 year of start
- Seasonal flu vaccination or declination during flu season
- Measles, mumps, rubella, hepatitis B, and varicella immunization/documentation requirements
- Skills checklist
- Competency exam
- Medication test/pharmacology competency exam, as applicable by profession
- National tech certification verification, as applicable
- State license verification
- Annual TB questionnaire
- Annual skills checklist
- Annual background and exclusion screenings
- Annual HHS/OIG screening
- Annual EPLS/GSA/SAM screening

- N95 mask fitting will be completed onsite
