Required Qualifications

• Valid Physical Therapist license in North Carolina
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• Experience in an Outpatient setting

Compliance / Onboarding

• Government-issued photo ID required
• Background check required within 1 year of start, covering all locations lived/worked/in school in the past 7 years
• EPLS/GSA/SAM screening required within 30 days of start and at each renewal/extension
• HHS/OIG screening required within 30 days of start and at each renewal/extension
• NPI number required and verified before start
• Primary source verification of state license required
• Two clinical references required; one must be from a supervisor or higher
• Complete application/resume required
• Skills checklist required annually

Health Requirements

• Physical exam required within 1 year of start
• Tdap required within 10 years of start
• Negative TB test or chest X-ray required, with annual update
• Measles, mumps, rubella, hepatitis B, and varicella documentation required
• 10-panel drug screen required within 30 days of start
• Flu vaccine or approved medical/religious exemption required during flu season

• Start date is mid to late September
• Candidates with approved flu exemptions must wear a mask in patient care areas
