Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Average patient load: 5-8 patients per day.
- Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
- Work involves home visits with periodic office meetings.
- Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
- Willingness to travel up to 75 miles from home clinic; estimated weekly travel 150-170 miles.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
- Weekend rotation: 1-2 times per month.
- On-call: One weekend per month plus backup call 3-4 times per month.
- Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).
- EMR system used: PointCare.
- Dress code: Scrubs.
Required Qualifications
- Valid RN license in Maine.
- Minimum 1 year of hospice nursing experience within the last year.
- Experience working with adult patients in a hospice and/or home health setting.
- BLS certification issued by the American Heart Association or American Red Cross.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Proficiency with HomeCare HomeBase (HCHB).
- Current driver’s license and reliable transportation.
