Job Description

Provide and coordinate home nursing care according to physician orders and agency policies.
Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Average patient load: 5-8 patients per day.
Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
Work involves home visits with periodic office meetings.
Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
Willingness to travel up to 75 miles from home clinic; estimated weekly travel 150-170 miles.
Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
Weekend rotation: 1-2 times per month.
On-call: One weekend per month plus backup call 3-4 times per month.
Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).
EMR system used: PointCare.
Dress code: Scrubs.

Required Qualifications

Valid RN license in Maine.
Minimum 1 year of hospice nursing experience within the last year.
Experience working with adult patients in a hospice and/or home health setting.
BLS certification issued by the American Heart Association or American Red Cross.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Proficiency with HomeCare HomeBase (HCHB).
Current driver’s license and reliable transportation.
