Job Description

• Provide individualized care to residents in a skilled nursing facility (SNF).
• Collaborate with nursing staff, residents, families, and other departments.
• Perform resident assessments, including new admissions and those who are ill, injured, or have fallen.
• Receive and give reports on residents' conditions to ensure continuity of care.
• Document assessments according to facility policy.
• Notify nurses of any changes in residents' conditions or needs.
• Assist with direct patient care and promptly respond to call lights.
• Float to other units as necessary.
• Work some weekends and holidays.
• Maintain good working relationships with co-workers, residents, families, and physicians.

Required Qualifications

• Valid CNA license in California
• Minimum of 1 year of Long Term Care experience within the last year
