Case Management Assistant
Location: Rancho Mirage, California
Employment Type: Non-Clinical Administrative
Shift: Monday to Friday, 8-hour days (8:00 AM – 4:30 PM)
Work Setting: Onsite
Contract Length: 4 weeks (with possibility of extension)
Job Description
- Ensure compliance with Code of Conduct and organizational compliance policies; report suspected violations.
- Assist with discharge referrals to outside agencies including skilled nursing, sub-acute and acute care facilities, transitional care, rehab services, home health agencies, and durable medical equipment providers.
- Facilitate acquisition of insurance information and authorizations prior to patient discharge.
- Complete and send all necessary post-discharge paperwork to providers as directed.
- Confirm agency/facility acceptance and transfer times; arrange transportation as directed; communicate updates to Case Managers, Social Workers, or Nurse Navigators.
- Perform phone calls and faxes to complete Medi-Cal 10-bed call list and other bed searches; document results in medical records.
- Communicate bed search results and barriers to acceptance to appropriate team members for patient/family communication.
- Prepare patient packets by copying medical records, printing reports, and collating required information for transfers.
- Maintain and update community resource lists.
- Assist with Medicare IMM letters distribution and signature collection, as applicable.
- Support Medicare appeals by gathering information, faxing to Quality Improvement Organizations, tracking outcomes, and communicating with the team.
- Schedule post-discharge primary care provider appointments for high-risk patients to help prevent readmissions.
- Manage voicemail messages and assist coworkers to promote efficiency and quality service.
- Prioritize workload to complete daily assignments within scheduled shifts.
- Stay informed about payer reimbursement practices and regulations affecting patient care and discharge plans.
- Perform other duties as assigned.
Required Qualifications
- Valid Registered Nurse (RN) license in California
- Minimum of 1 year of case management experience, including experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
