Job Summary: The Case Management Assistant supports the case management team by facilitating discharge referrals, ensuring proper documentation, and coordinating with external agencies and providers. Responsibilities include assisting with insurance authorizations, preparing patient packets, maintaining community resource lists, and supporting Medicare-related processes. The role requires effective communication, problem-solving skills, and the ability to prioritize tasks to ensure efficient and quality service delivery.

Required Qualifications:

• High school diploma, GED, or higher-level degree
• Knowledge of payer types, including Medicare, Medi-Cal, and private payers
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and relevant software applications
• Epic knowledge
• Strong written and verbal communication skills
• Problem-solving skills to identify issues and formulate effective solutions
• Ability to effectively communicate in a positive and professional manner

Preferred Qualifications:

• Associate's degree
• Medical Assistant, Certified Nursing Assistant, Medical Terminology, or other health-related certification

Additional Information:

• Pay range: $19.72 - $29.95 per hour
• May include benefits such as stipends (for clinical traveler workers), medical, dental, vision, and 401K
