Location
- Salinas, California
Job Summary
- Perform Utilization Review and Discharge Planning for assigned patients under direction of management.
- Analyze medical records for medical necessity, level of care, and appropriate utilization of hospital services.
- Assist patients and families with discharge planning and aftercare.
- Assess patient needs, identify resources, and develop formal discharge plans.
- Collaborate with hospital personnel to manage care delivery and monitor length of stay.
- Screen patients for discharge needs and facilitate discharge teaching.
Specialty-Specific Responsibilities
Neonatal Intensive Care Unit (NICU)
- Ensure appropriate discharge information and follow-up care for NICU infants, including referrals to specialty programs and home health services.
- Arrange durable medical equipment as ordered.
- Develop and implement neonatal discharge planning standards.
- Maintain expertise through ongoing education and literature review.
Pediatric
- Ensure discharge information is communicated to primary care physicians and community agencies.
- Provide culturally and linguistically appropriate written discharge instructions covering diagnosis, medications, follow-up appointments, and home medical treatments.
- Teach caregivers about medical needs and use of technology for post-discharge care.
- Coordinate multidisciplinary healthcare team members in discharge planning.
Unit Details
- 3 Recovery bays
- 2 Procedure rooms plus travel to ICU, Diagnostic Imaging, Operating Room, etc.
- Nurse-to-patient ratio: 1:3
Required Qualifications
- Minimum 3 years acute medical-surgical nursing experience.
- At least 1 year experience in utilization review, case management, or discharge planning in an acute care hospital.
- Endoscopy experience required.
- Experience in Progressive Care, Critical Care, and/or Emergency Room.
- Skills in airway management, EKG interpretation, moderate sedation, sedatives and reversal medications, and Phase II recoveries.
- Recent charge nurse experience.
- Flexibility to float within cluster or similar departments is highly desirable.
- Epic electronic health record experience: 6 to 12 months within the past year.
Shift Details
- Department operating hours: 06:30 – 20:30.
- Traveler shift hours are variable.
- On-call expectation on weekdays and weekends.
Contract Length
- Approximately 13 weeks (90 days).

Problem is they will call you at 6, tell you you’re cancelled 7-11 and expect you to be on stand by until 11 at which time they can (and usually do) cancel for the whole shift. - No OT
