RN Case Manager - Contract Position
Location: Tuba City, Arizona
Contract Length: 13 weeks
Job Summary
- Serve as an Outpatient/Inpatient Nurse Case Manager focusing on specialty clinics requiring case management attention.
- Facilitate care transitions from admission through discharge, ensuring continuity of care and alignment of services.
- Act as a resource for nursing, physicians, and ancillary departments on transitional and regulatory issues.
- Work with culturally diverse patient populations facing barriers such as illiteracy, welfare dependency, domestic violence, substance abuse, and mental health issues.
- Collaborate with an interdisciplinary team to assess care plans, monitor progress, and recommend safe and efficient hospitalizations.
- Evaluate patients in the Emergency Department for medical necessity and care coordination needs, including admission status determination and outpatient discharge planning.
- Coordinate care for patients with abuse, chronic pain, and frequent utilization issues.
- Interact with patients of all ages, from newborn to geriatric, including those with mental health challenges.
Work Schedule
- Monday to Friday, 8:00 AM to 5:00 PM
- Weekend rotation with on-call responsibilities
Required Qualifications
Education
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Licensure & Certifications
- Valid Registered Nurse (RN) license in Arizona
- Current Basic Life Support (BLS) certification issued by the American Heart Association
Experience
- Minimum 3 years of case management experience, including experience within the last year
- Experience in both inpatient and outpatient settings
- Knowledge and experience with contract health processes
- Experience with utilization review and utilization management processes
Additional Information
- Fingerprinting must be submitted and cleared prior to start
- Hospital rental property is very limited; shared contractor housing only (no pets or family allowed)
- Orientation occurs every 2 weeks
- COVID and flu vaccination exemptions may be accepted after submission of a declination form
- Housing application must be submitted 2 weeks prior to start
- Required certifications may include ACLS, BLS/BCLS, PALS, NRP, ATT (verify specific requirements during onboarding)

- Diverse community with Anglo, Hispanic, and African American populations
- The healthcare facility serves a large rural area with a variety of inpatient and outpatient services including emergency, dental, ophthalmology, orthopedics, OB/GYN, and surgical specialties
This position does not require an Arizona state nursing license.
