Description
American Traveler is seeking an experienced RN Case Manager for a 13-week travel assignment at a JCAHO-accredited regional health center in Northwestern Arizona, serving a culturally diverse Native American community.
Details
- Inpatient and outpatient case management setting within a 73-bed JCAHO-accredited regional health center
- Serves a 4,400 square mile referral area covering the western Navajo and Hopi Reservations
- Caseload includes high-risk patients across specialty clinics, the ED, and inpatient units with complex social and medical needs
- Patient population spans all ages (newborn to geriatric) from culturally diverse Native American and other backgrounds
- Responsible for evaluating ED patients for medical necessity and care coordination needs
- Monday through Friday, 8am–5pm schedule with weekend rotation and on-call requirements
- 13-week contract
Requirements
- Active AZ RN license or compact RN license currently in hand
- Current BLS certification (American Heart Association)
- Minimum 3 years of clinical nursing experience, including at least 1 year in an outpatient or inpatient setting
- Demonstrated knowledge of case management, utilization review, and contract health processes
- Associate's or Bachelor's Degree in Nursing
- Fingerprinting must be completed and cleared prior to start
Additional Information
- Collaborates with an interdisciplinary team to assess care plans, monitor progress, coordinate transitions from admission through discharge, and ensure continuity of care
- Facilitates post-hospital referrals to continuing care agencies and serves as a resource for nursing, physicians, and ancillary departments on transitional and regulatory issues
- Works with patients who have complex social barriers including substance abuse, mental health challenges, domestic violence, and chronic pain
- Assists individuals with mental health issues in accessing and navigating available community resources
- Orientation is offered on a bi-weekly cycle
- COVID and flu vaccine exemptions may be accepted upon submission of a declination form
- Facility is located in a very rural area — shared contractor housing may be available but is not guaranteed; a housing application must be submitted at least 2 weeks prior to start
- Housing does not allow pets or family members per facility policy
