Physical Therapist – Home Health

Location: Missoula, Montana
Employment Type: Contract
Contract Length: 13 weeks
Schedule: Full-Time, typically Monday through Friday, 8am–5pm, with possible on-call, weekend, and holiday coverage
Hours per Week: 40 hours

Job Summary

Provide evaluation and rehabilitation interventions in patients' homes, including management and supervision of patient care and education for patients and families.

Key Responsibilities

Evaluate patients and establish individualized plans of care for the home environment.
Coordinate care with other disciplines and obtain orders for additional services as needed.
Deliver direct patient care based on care plans and physician orders.
Re-evaluate patient progress and adjust treatment frequency and duration accordingly.
Communicate effectively with physicians, staff, and schedulers.
Document all skilled services provided.
Maximize consistency of care through effective scheduling.
Oversee services provided by Physical Therapist Assistants, if applicable.
Perform other duties as assigned.

Required Qualifications

Valid Physical Therapist license in Montana
Basic Life Support (BLS) certification

Additional Information

Day shift with possible on-call duties.
Work involves travel to patient homes within the service area.
