Job Description
- Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
- Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
- Enhance patient management quality and satisfaction, promoting continuity of care and cost-effectiveness.
- Integrate case management, utilization review, and discharge planning functions.
- Serve as a patient advocate addressing holistic patient needs and preferences.
- Work schedule: Monday to Friday, 8:00 AM to 4:30 PM (40 hours per week).
Required Qualifications
- Valid RN license in Washington.
- Associate of Science in Nursing or Associate Degree in Nursing.
- Minimum 1 year of case management experience, including experience within the last year.
- Experience in a Long Term Acute Care setting.
- Acute hospital case management experience.
- BLS certification issued by the American Heart Association.
- Completion of Suicide Prevention Training.
