Registered Nurse - Case Management (Travel)

Location

Seattle, Washington

Travel assignment
Contract length: 13 weeks

Job Description

Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
Enhance patient management quality and satisfaction, promoting continuity of care and cost-effectiveness.
Integrate case management, utilization review, and discharge planning functions.
Serve as a patient advocate addressing holistic patient needs and preferences.
Work schedule: Monday to Friday, 8:00 AM to 4:30 PM (40 hours per week).

Required Qualifications

Valid RN license in Washington.
Associate of Science in Nursing or Associate Degree in Nursing.
Minimum 1 year of case management experience, including experience within the last year.
Experience in a Long Term Acute Care setting.
Acute hospital case management experience.
BLS certification issued by the American Heart Association.
Completion of Suicide Prevention Training.
