Job Opportunity: Physical Therapist - Home Health
Location: Chesapeake, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
The Home Health Physical Therapist provides medically prescribed physical therapy treatments to patients with injuries or muscle, nerve, joint, and bone diseases. The goal is to restore function, relieve pain, and prevent disability. Responsibilities include patient education, discharge planning participation, documentation, and directing patient participation in therapeutic tasks.
Work Schedule
- Monday to Friday, 8:00 AM to 5:00 PM
- Weekend work for orthopedic cases only (every 4th or 5th weekend)
Requirements
- Specialty: Therapy/Rehabilitation (Physical Therapy)
- Years of Experience: Minimum 1 year (open to first-time travelers)
- Prior Travel Experience: Not required
- Licenses/Certifications:
- Virginia Physical Therapy State License (required)
- CPR certification (required)
- Preferred Experience:
- Home Health experience preferred
- Experience with MatrixCare or Epic (preferred but not required)
Patient Load
- 6 to 7 weighted visits per day
Key Facility Policies
- Zero tolerance for any license or background hits
- Mileage reimbursed at IRS rate
Documentation & Compliance Requirements
- I9 verification
- Government-issued photo ID
- E-Verify compliance
- 7-year criminal background check within 1 year of start, updated annually
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated for renewals
- Proof of diploma or transcript (as applicable)
- Fingerprinting required (Virginia VSP National Criminal Background Check) effective 1/1/2026, to be completed within the first week of contract
- Physical exam within 1 year of start, updated annually
- Fit test for N95 mask within 1 year, updated annually (PAPR not allowed)
- Negative TB test or chest X-ray within 1 year, with additional requirements if history of positive TB
- Required immunizations and titers:
- Tetanus/Diphtheria/Tdap (preferred for most units)
- Measles, Mumps, Rubella, Hepatitis B, Varicella titers or declinations as applicable
- Negative 10-panel drug screen within 12 months
- Seasonal flu vaccination required or approved exemption (mask required if exempt)
- COVID vaccine preferred but not required
Additional Requirements
- Proof of 2 professional references
- Completed employment application/resume
- Unit-specific skills checklist, updated annually
- Competency exams and medication tests as applicable
- Orientation safety manual acknowledgment
- Pass/fail testing for EKG/medication (for RNs, if applicable)
Notes
- Open to first-time travelers; prior travel experience is not required.
- Weekend work is limited and specific to orthopedic cases.
- Facility maintains strict compliance and documentation standards to ensure patient and staff safety.
