Job Description

• Provide outpatient Speech-Language Pathology (SLP) services in a Neuro Rehabilitation environment.
• Preference for candidates with prior travel experience, but open to first-time travelers with outpatient experience.

Required Qualifications

• Valid Speech Language Pathologist license in Texas.
• Minimum of 1 year of Speech Language Pathologist experience.
• Speech Language Pathologist experience within the last year.

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID).
• Social Security Card.
• I-9 or Attestation.
• Background Check: 7-year check required within 30 days of start.
• E-Verify or Attestation.
• Education Verification and/or Diploma.
• Professional license verification within 30 days of start.
• Clear, passport-style photo for badge.
• Employment application or resume.
• Two professional references within the last 3 years.
• Skills checklist updated annually.

Certifications

• Basic Life Support (BLS) – AHA or ARC accepted.
• Advanced Cardiac Life Support (ACLS) – AHA or ARC accepted.
• Other certifications as applicable based on job duties and facility requirements.

Health & Safety Requirements

• Physical exam within 1 year of start, completed within 72 hours of contract signing.
• Fit test within 6 months prior to start; annual updates required.
• Negative TB test or chest X-ray within 1 year of start; TB questionnaire annually.
• Required immunizations or proof of immunity:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (positive titer or vaccination series/declination form)
  • Varicella
  • Tetanus/Diphtheria/Tdap (within 10 years)
• Drug testing within 30 days of start.
• Seasonal flu vaccination or facility-specific declination during flu season.
• COVID-19 initial vaccination series required (boosters not required).

Facility-Specific Requirements

• Completion of various facility forms prior to start, including:

  • Health record form
  • Code of conduct and compliance form
  • Confidentiality statement
  • Information system security agreement
  • Non-employee orientation update
  • Traveler orientation
  • Vaccine exemption form (if applicable)
  • Hepatitis B declination form (if applicable)

Additional Notes

• Legal name must match exactly on all license documentation, IDs, social security card, and certifications.
• Background checks and certifications must be current and meet facility standards.
• Pass/fail testing on medication administration and other competencies may be required during orientation.
