Case Manager (Registered Nurse) - Travel/Contract
Location: Atlanta, Georgia
Shift: Day (Monday-Friday, 8:00 AM - 4:30 PM)
Hours: 40 hours per week (5x8 hour shifts)
Contract Length: 12 weeks
Unit Details
- Assess, evaluate, and drive discharge planning.
- Coordinate services to ensure appropriate level of care.
- Initiate, evaluate, and re-evaluate treatment plans daily.
- Monitor patient progress and barriers to discharge.
- Follow Medicare regulatory guidelines regarding patient choice, IMM notices, and MOONS.
- Floating within facility/scope of practice as needed.
- No weekend or on-call requirements.
Required Qualifications
- Valid Registered Nurse (RN) license in the state of Georgia.
- Minimum of 3 years of case management experience.
- Case management experience within the last year.
- Completion of at least one travel assignment.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Compliance & Onboarding
- Full compliance with all documentation and credentialing required to start on time.
- Maximum Return to Office (RTO) time: 7 days.
- Orientation hours are non-billable and vary by contract length (e.g., 8 hours for 8-week assignment).
- Must work major holidays if they fall during assignment (expectation varies by number of holidays).
Background & Screening Requirements
- Background check required within 30 days of start, including:
- National and county criminal searches
- Sex offender registry check
- Education verification of related degree
- Clinical screening to determine fit and eligibility before final approval.
Health & Immunization Requirements
- Negative TB test (PPD, QGold, or TSpot) within 12 months; if past positive, additional documentation required.
- TB Questionnaire updated annually, required for all candidates.
- Physical exam within 12 months of start.
- Drug screen (10-panel) within 60 days of start; instant drug screen acceptable.
- Immunizations or proof of immunity required:
- 2 MMR vaccines or positive titers for Rubeola (Measles), Mumps, Rubella
- 2 Varicella vaccines or positive titer
- Hepatitis B surface antibody titer or signed declination
- TDAP highly recommended but not required
- Flu vaccine or declination required during flu season (Nov 1 - Mar 31).
Competency & Testing Requirements
- Medication/Pharmacology competency exam required.
- Color blind testing required; alternative assessment available for blood collection tasks.
- Dysrhythmia/EKG competency exam required for telemetry and related units; up to 3 attempts allowed.
- Additional competency exams may be required based on unit assignment.
Documentation & Policies
- Must review and sign Code of Conduct and Dress/Professional Appearance policies prior to start.
- Skills checklist updated within 12 months required.
- Verification of most recent work history required; can be pending at start with proof of processing.
- Two clinical references required.
Additional Notes
- Locals are accepted at the same rate.
- Number of non-billable orientation hours depends on contract length.
- Floating assignments may occur within the facility as needed.
- Candidate must be prepared to work major holidays if they fall during the assignment period.
