Speech-Language Pathologist (SLP) - Travel/Contract Position

Location: Stockton, California
Assignment Length: 13 to 26 weeks preferred
Shift: Day shift, 5 days per week, 8 hours per day (40 hours/week)
Start Date: ASAP
Schedule Details:

May include occasional weekend coverage and floating
Cancellation notice: 14 days

Job Description

Provide speech-language pathology services in a skilled nursing facility setting.

Required Qualifications

Valid Speech Language Pathologist license in California

Required Documentation and Compliance

Government-issued photo ID (Driver’s License or State ID)
Emergency Contact Form (required prior to start)
Background checks including:
- 7-year residential/work/school history
- EPLS/GSA/SAM verification (annual)
- HHS/OIG verification (annual)
Physical exam within 1 year of start date
Negative TB test or chest X-ray within 1 year (with specific protocols for past positives)
Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccine, or history of chickenpox)
9-panel drug screen within 1 year of start (instant drug screen acceptable)
Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
Completion of facility-specific compliance quiz (Wage and Rehab Hour Quiz)
Annual skills checklist and competency exams as applicable (including medication/pharmacology competency)
National tech certification or state license verification as applicable

References

2 professional references required for travel, allied, or per diem acute care roles (within 3 years)
1 professional reference required for per diem non-acute care roles

Additional Notes

This is a travel/contract position with a preferred assignment length of 13 to 26 weeks.
Prior travel experience is not explicitly required.
Floating and occasional weekend coverage may be required.
