• Varicella: Proof of 2 immunizations, positive titer, or declination accepted
• Tdap: Proof of vaccination within past 10 years or declination accepted
• Influenza Vaccine: Required October - March unless exempted (medical/religious exemptions require approval)
• Drug Screen: 10-panel standard test within 30 days of start, updated annually; rapid drug screen accepted

Background & Compliance Checks

• 7-year County and National Criminal Background Check, SSN Trace, Employment Verification, National Sex Offender, OFAC, and VSOP within 6 months of first assignment; updated every 3 years
• Manual searches for OIG, EPLS, OFAC, National Sex Offender, and CA Medi-Cal Suspended and Ineligible Provider List within 30 days of start; updated annually
• Health Statement: Signed and dated within 1 year stating ability to work without restrictions
