Job Description

• Position involves working in a medical/surgical unit.
• Facility is part of a large hospital system in Wichita, KS.

Required Certifications and Licenses

• BLS (Basic Life Support): Must be American Heart Association (AHA) certified.
• Nursys Verification: Must be completed within 30 days prior to start. Any disciplinary actions require completion of a FACIS Questionnaire.
• Current Nursing License: Primary source verification within 30 days prior to assignment start. FACIS Questionnaire required if disciplinary actions exist.
• Ascension Color Vision Test: Completed prior to start and updated every 3 years.
• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly with a minimum 80% score.
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
• Ascension EKG Exam: Must be completed with a passing score of at least 80%, with only two attempts allowed.
• Ascension Onboarding Requirements Education Attestation: Must be completed pre-hire and uploaded.
• Ascension Nursing Contract Worker Onboarding: Completed onsite prior to start.
• Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
• MOAB Moderate Risk De-Escalation Training: Required.
• Ascension Cerner Test-Out: Must complete Cerner test results and training checklist with signed attestation.

Health and Immunization Requirements

• MMR: Documented proof of two MMR vaccines or positive IGG titer; no declinations accepted. If negative, two doses of MMR vaccine required four weeks apart.
• Fit Test: Required for all travelers; preferred prior to start but can be completed onsite. Annual update required.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Exemptions accepted with mask requirement.
• Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within one year showing no infection, and completed TB questionnaire. Annual updates required.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within 30 days prior to start.
• Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider. No declinations accepted.
• Tdap: Valid within 10 years; no declinations accepted.
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start and updated annually. Includes testing for 12 specified drugs with MRO review if applicable.

Professional and Background Requirements

• Driver's License/State ID: Must be current, match address on file, and comply with compact license rules if applicable.
• Ascension Profile (Resume): Required for submission.
• Professional References: At least two qualifying references prior to first assignment.
• Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender check; completed within 30 days of first assignment and updated annually or after breaks in service.
• EPLS (Excluded Parties List System) Check: Run within 90 days of start and rerun if break in service > 90 days.
• Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
• Education Verification: Verification of education relevant to clinical work within 90 days of start; must be rerun if break in service > 90 days.
• Confirmation of Assignment: Provided upon initial assignment and updated with each extension.
• Employment Verification: Third-party verification for past seven years; updated annually or after breaks in service.

Optional / Additional Requirements

• Ascension Unit Specific Checklist: Optional for onboarding but must be uploaded within a week by unit leader.
• Hiring Manager Evaluation: Post-assignment evaluation (optional).
• Name Change Documentation: Legal documentation if applicable (optional).
