Registered Nurse (RN) - Home Care Visiting Nurse

Location: Philadelphia, Pennsylvania
Employment Type: Temporary
Contract Length: 13 weeks

Job Description

• Provide safe, holistic care to patients with multiple co-morbidities and advanced chronic illnesses in a home care setting.
• Educate patients and families on self-management and provide ongoing support.
• Manage a caseload of approximately 6 patients per day, with a productivity target of 30 visits per week.
• Complete revisits, Start of Care (SOC), Recertifications (ROCs), recerts, and discharges.
• Work territory limited to specific zip codes within Philadelphia County.
• Mileage reimbursement provided for travel within the territory.

Requirements

• Experience:

  • Minimum 1+ years recent Home Health experience (required)
  • Case management experience (required)
  • Previous travel nursing experience (highly preferred)
  • IV and venipuncture experience (highly preferred)
• Technical Skills:

  • Proficiency with Homecare Home Base EMR (required)
• Licenses & Certifications:

  • Active Pennsylvania RN license (must be current at time of submission)
  • BLS (AHA) certification (must be active)
• Other:

  • Clear driving record, active driver’s license, and current auto insurance (proof required within 48 hours of offer acceptance)
  • Must provide 2 supervisory references from the last 2 years
  • Skills checklist specific to nursing discipline (less than 1 year old)

Work Conditions & Additional Information

• Work week runs Sunday through Saturday.
• Overtime, holiday, and callback pay at 1.25x with a minimum of 2 hours for callbacks.
• Redeemer reserves the right to cancel 1 shift per pay period.
• Orientation: 8 hours non-billable.
• Mileage reimbursed at $0.62 per mile.
• Locals are accepted for this position.

Summary

This role is ideal for an experienced Home Health RN with strong case management skills, comfortable working independently in a home care environment, and able to manage a moderate patient load within Philadelphia County. The position requires reliable transportation and the ability to navigate specific zip codes for patient visits.
