Job Opportunity: LVN / LPN - Rochester, New York (Travel/Contract)

Position Overview

Experience Requirements

• Must have a valid New York LPN license.
• Hospital experience within the last 2 years is required.
• Must be able to take own patient assignments with RN support.

Health & Immunization Requirements

Candidates must complete health-related requirements and lab work within 5 days of signing the contract:

• Physical exam within 1 year of start (updated annually)
• Fit mask test within 1 year of start (specific mask types accepted)
• Negative TB test or chest X-ray within 1 year of start (with additional requirements if positive)
• TB questionnaire (acceptable in lieu of annual TB test renewal)
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (Surface AB positive titer, completed series, or declination)
  • Varicella
• Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
• Seasonal flu vaccination or signed declination (required during flu season)
• Snellen eye exam within 1 year of start (not required annually)
• COVID vaccination copy preferred but not required

Competency & Facility Requirements

• Medication test/pharmacology competency exam as required.
• Color blind testing (one-time requirement).
• Competency exam as required by specialty.
• Orientation safety manual acknowledgement.
• Photo for badge (passport-style with solid background).
• Facility-specific modules and attestations to be completed prior to start.
