Overview

Travel/contract role in Oakland, California
Day shift: 5x8 hours, 8:00–16:30
40 hours per week
Contract length: 13 weeks

Required Qualifications

Valid RN license in California
Case Management (1 year of experience)
Case Management experience within the last year
Associate of Science in Nursing / Associate Degree in Nursing

Required Certifications

ACLS
BLS
PALS
NRP
Chemotherapy certification
TNCC
EFM

Required Health and Compliance Items

Government-issued photo ID or passport
Background check
EPLS/GSA/SAM check
HHS/OIG check
Orientation safety manual acknowledgement
Badge photo and badge photo agreement
Facility competencies completed on first day
Physical exam within 90 days of start
Fit mask testing within 90 days of start
Tdap within 10 years of start or signed declination
Negative TB test or chest X-ray, with annual update
TB questionnaire, updated annually
Proof of immunity or vaccination for measles, mumps, rubella, and varicella
Hepatitis B titer or declination
Oral saliva drug screen within 30 days of start
Medical and employee information release form
Flu vaccination or declination on file by 11/06
Color blind exam within 90 days of start

Role Focus

Assess discharge planning and care coordination needs
Develop and update individualized discharge plans
Coordinate post-acute services and authorizations
Participate in interdisciplinary rounds
Educate patients and families on discharge plans and next steps
Document discharge planning activities and barriers
Escalate complex discharge barriers as needed
