Employment Type: Long-Term Assignment (13 weeks)
Shift: 7:00 PM – 7:30 AM

Drug Screen: 10-panel drug screen completed within 30 days of start (no THC).
Background Checks: Federal, nationwide, and county criminal background checks from last 7 years, including SSN verification, dated within 30 days of start.
Education Verification: Highest academic degree or diploma verified.
Professional Forms:
- Signed Code of Conduct form.
- Confidentiality and Information Security Agreement.
- Conflict of Interest Disclosure form.
- Integrity and Audit Services form.
- Criminal Background Check report.
- EPLS and OIG exclusion reports dated within 30 days.
- NSO (National Sex Offender) check dated within 30 days.
Specialty Competency Testing: Specialty-specific competency exam completed within the last year.
Joint Commission CORE Exam within the last year.
System Access Form completed and uploaded one week before start.
Skills Checklist provided by agency for clinical contractors.
