Onboarding and Credential Requirements

Licenses & Certifications

• License/Certificate Verification: Primary source verification required within 30 days of start date. MA Conditional Licenses are not accepted.
• ARRT Certification required.
• MGB Education Verification: Verification of highest education level required.
• MGB Resume: Must include past 7 years of work experience with city & state.
• MGB References: Two recent references required, at least one supervisor or above in the skillset.
• MGB Skills Checklist: Required for submission.
• MGB Candidate Submission Coversheet: Required for submission.

Health & Immunization Requirements

• Fit Test: N95 respirator fit test within 365 days of start date, updated annually. Must be completed prior to start; no onsite testing available.
• MMR Vaccine or Proof of Immunity: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or blood test proof of immunity.
• Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
• TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date, updated if >6 month break.
• Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date, updated if >6 month break.
• Respirator Medical Clearance Attestation: Required if fit test is required.
• Hepatitis B Vaccine or Immunity: Strongly preferred but optional.
• Tdap Vaccine: Preferred, within last 10 years.
• Positive PPD with Negative Chest X-Ray: Required if applicable.
• MGB TB Symptom Survey (if Positive PPD): Optional but recommended.

Background and Compliance Checks

• MGB Criminal Background Check: Within 45 days prior to start, updated if >6 month break. Includes international searches if applicable.
• MGB Massachusetts CORI: Criminal history check for MA within 30 days prior to start, updated if >6 month break.
• MGB SSN Trace: Must be run with criminal check, covering last 7 years of addresses/aliases.
• MGB National Sex Offender Check: Within 30 days of start, updated annually.
• MGB Exclusion Checks: Within 30 days of start and annually; includes OIG, GSA, MassHealth, and NH Medicaid exclusion lists.

Additional Requirements

• MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions, updated if start date changes.
• MGB NWH Self-Study Orientation & Attestation: Required for onboarding.
