RN Care Coordinator

Overview

  • Contract length: 13 weeks

Role Summary

  • Manages a caseload of patients and coordinates care to support clinical and financial outcomes.
  • Works with an interdisciplinary team and uses patient assessment, care guidelines, protocols, payer regulations, and therapy response to evaluate the episode of illness from pre-admission to post-discharge.
  • Supports quality, cost, and satisfaction improvement efforts and helps mobilize resources to improve care efficiency.

Required Qualifications

  • Valid RN license in Massachusetts.
  • Case Management experience within the last year.
  • 1 year of Case Management experience.
  • Associate of Science in Nursing / Associate Degree in Nursing.

Onboarding Requirements

  • Color badge photo taken within the past 6 months.
  • Fit test for N95 within 365 days of start date, with annual updates.
  • Respirator medical clearance attestation, if fit testing is required for the skill.
  • MMR documentation or proof of immunity.
  • TB screening documentation within 3 months of start date.
  • Physician health statement within 365 days of start date.
  • Criminal background check.
  • Massachusetts CORI check.
  • Education verification.
  • SSN trace.
  • National sex offender check.
  • Exclusion checks.
  • Confirmation of assignment/POI form.
  • Beeline attestation.

Optional

  • Chest X-ray documentation if prior TB testing was positive.
  • Name change documentation if legal name differs from credential documentation.
