Overview

Role Summary
- Manages a caseload of patients and coordinates care to support clinical and financial outcomes.
- Works with an interdisciplinary team and uses patient assessment, care guidelines, protocols, payer regulations, and therapy response to evaluate the episode of illness from pre-admission to post-discharge.
- Supports quality, cost, and satisfaction improvement efforts and helps mobilize resources to improve care efficiency.
Required Qualifications
- Valid RN license in Massachusetts.
- Case Management experience within the last year.
- 1 year of Case Management experience.
- Associate of Science in Nursing / Associate Degree in Nursing.
Onboarding Requirements
- Color badge photo taken within the past 6 months.
- Fit test for N95 within 365 days of start date, with annual updates.
- Respirator medical clearance attestation, if fit testing is required for the skill.
- MMR documentation or proof of immunity.
- TB screening documentation within 3 months of start date.
- Physician health statement within 365 days of start date.
- Criminal background check.
- Massachusetts CORI check.
- Education verification.
- SSN trace.
- National sex offender check.
- Exclusion checks.
- Confirmation of assignment/POI form.
- Beeline attestation.
Optional
- Chest X-ray documentation if prior TB testing was positive.
- Name change documentation if legal name differs from credential documentation.
