Description
American Traveler is hiring an RN Care Coordinator for a Case Management position requiring an active MA RN license and Epic charting experience.
Details
- Case Management/Care Coordination unit within a hospital setting
- Manages a caseload of patients across the episode of illness from pre-admission to post-discharge
- Day shift schedule from 07:00-15:30
- Uses Epic for charting
Requirements
- Active MA RN license required (no conditional or pending licenses accepted)
- AHA-issued certifications only
- Flu vaccine required with no religious or medical exemptions allowed

- Coordinates care prescribed by an interdisciplinary team using patient assessment, care guidelines, protocols, and payer regulations
- Participates in ongoing evaluation of practice patterns to improve quality, cost, and satisfaction outcomes

- Local candidates must reside at least 60 miles from Boston to qualify for standard travel rate
