Shift:

• 5 days per week, 8-hour day shifts
• Occasional weekend coverage and floating as needed

Job Description

Seeking a Physical Therapist for a skilled nursing facility (SNF) setting. The assignment requires working 36 hours per week with a start date ASAP.

Required Qualifications

• Valid California Physical Therapist license
• Minimum 1 year of experience in skilled nursing facility (SNF) specialty preferred; new graduates will be considered
• Current Basic Life Support (BLS) certification (American Heart Association or American Red Cross)
• Advanced Cardiac Life Support (ACLS) certification preferred
• Pediatric Advanced Life Support (PALS) certification preferred
• Must provide government-issued photo ID
• Must complete standard background checks including 7-year work/living history and national registries
• Required health documentation including:

  • Physical exam within 1 year of start
  • Negative TB test or chest x-ray within 1 year
  • Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella
  • Drug screening (9 panel) within 1 year
  • Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
• Required competency exams and medication/pharmacology competency as applicable
• Professional references required (2 for travel/allied/per diem acute care; 1 for per diem non-acute care)

• May require occasional weekend shifts and floating to other units
