Case Manager (Registered Nurse) - Travel/Contract

Location: Oakland, California
Contract Length: 13 weeks


Job Summary

  • Full-time day shift: 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
  • Every other weekend schedule
  • Focus on inpatient acute case management within an acute care hospital setting
  • Responsible for care coordination and care transitions planning throughout the acute care patient experience
  • Collaborates with physicians, utilization managers, social workers, bedside nurses, and interdisciplinary teams to ensure timely patient progression and appropriate level of care
  • Aims to prevent unnecessary admissions and readmissions by managing complex clinical and social situations efficiently

Key Responsibilities

  • Conduct initial and ongoing patient assessments including medical, psychosocial, and financial information
  • Lead healthcare team in developing cost-effective care coordination plans
  • Oversee case management assistants for post-acute care service preferences
  • Perform utilization management by reviewing medical records to confirm level of care (LOC) requirements and expedite transitions
  • Monitor length of stay and collaborate with financial counselors on inpatient stay denials
  • Ensure compliance with Medicare Important Message delivery timelines
  • Participate in patient rounds and interdisciplinary team meetings for timely transitions
  • Develop and communicate transition plans involving patients, families, payers, and community resources
  • Screen for 30-day readmissions and identify barriers to care progression
  • Address social determinants of health and complex psychosocial issues with appropriate referrals
  • Maintain documentation in electronic health records (EHR) and comply with regulatory requirements (CMS, Joint Commission, Conditions of Participation)
  • Actively participate in department operations, process improvements, and effective communication

Required Qualifications

  • Valid Registered Nurse (RN) license in California
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum 5 years of case management experience, including acute hospital case management
  • Case management experience within the last year
  • Experience in an inpatient setting
  • Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
  • Advanced Cardiovascular Life Support (ACLS) certification issued by American Heart Association or American Red Cross

Background and Compliance Requirements

  • 7-year background check including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening
  • EPLS/GSA/SAM and HHS/OIG checks, updated annually
  • Drug screening: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids due to California law)
  • Physical exam within 1 year of start, including clearance for work
  • TB screening: Negative QGold or TSPOT within 90 days or 2-step PPD testing with annual renewal
  • Immunizations and titers required:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Measles (Rubeola), Mumps, Rubella (MMR)
    • Hepatitis B series or declination
    • Varicella
    • Seasonal flu vaccination during flu season (Nov-April) or signed declination
  • Fit mask test within 1 year, with accommodations for religious exemptions or failed tests requiring PAPR training
  • Respiratory safety clearance required annually
  • References: 2 professional references within 3 years (Aya requirement, not client)
  • Full 7 years of work history with no gaps over 30 days, verified employment history

Certifications and Competencies

  • BLS (Basic Life Support) required
  • ACLS, PALS, CCRN, TNCC, NRP, EFM, CPI, NIH Stroke Certification, and other certifications as applicable may be required based on position specifics (verify with recruiter)
  • Medication/Pharmacology competency exams and dysrhythmia/EKG competency exams may be required depending on unit assignment
  • State license primary source verification within 30 days of start
  • NURSYS verification for RN license within 30 days of start (may not be required for case managers)

Facility-Specific Documents and Orientation

  • Must review and sign facility-specific documents including job description, standards of business conduct, confidentiality agreements, HIPAA agreement, and orientation booklet with post-test
  • Complete facility-required attestations such as conditional offer of employment and religious accommodation forms if applicable
  • Complete vendor training and code of conduct documents for the contract year prior to start
