Job Opportunity: Hospice Registered Nurse Case Manager

Location: Aldie, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Summary

• Role: Hospice Case Manager (Registered Nurse)
• Patient Load: Approximately 5-7 patients per day (geriatric, end-of-life care)
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 5:00 PM)
• Guaranteed Hours: 40 hours per week
• Territory: Up to a 60-mile radius from the office; flexibility with territory expected
• Dress Code: Scrubs of any color; jeans are not allowed
• Documentation and communication must be timely and accurate
• Device Provided: iPad (cell phone use not allowed)
• Mileage reimbursed at IRS standard rate

Required Qualifications

• Valid Registered Nurse (RN) license in Virginia
• Minimum of 1 year of hospice experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Compliance and Background Requirements

• Government-issued photo ID required
• 7-year background check within 30 days of start, updated annually
• Virginia State Police background check required regardless of candidate’s state
• E-Verify or attestation required
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
• Fingerprinting clearance per Florida AHCA Level II requirements (if applicable) must be completed prior to start
• Physical exam within 6 months prior to start, confirming freedom from communicable diseases
• Fit test for N95 mask within 1 year of start (can be completed onsite)
• Negative 10-panel drug screen within 30 days of start
• Immunizations and titers required:

  • TDAP within 10 years
  • Positive titers and boosters as needed for Measles, Mumps, Rubella, Hepatitis B, Varicella
• Annual skills checklist and competency exams as applicable
• Flu vaccine or facility-specific declination required annually (flu season Oct 1 – Apr 1)
• COVID vaccine or facility-specific declination required

Additional Requirements

• Must complete facility-specific forms prior to start, including:

  • Contractor info form
  • Confidentiality and security agreement
  • Immunization statement
  • TB symptom survey
  • Sexual abuse policy
  • Code of conduct
  • Conflict of interest form
  • Flu and COVID vaccine declination forms if applicable
• Passport-style photo required for badge issuance
• Alzheimer’s training (1 hour) required for candidates working at PACE facilities
