Position Overview

  • Employment Type: Travel/Contract
  • Location: Sacramento, CA
  • Specialty: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
  • Patient Population: Adult psychiatric patients with diagnoses including ADHD, Bipolar Disorder, Autism Spectrum, Eating Disorder, PTSD, Anxiety Disorder, Depression, Major Depressive Disorder, Psychotic Disorder, Oppositional Defiant Disorder, Substance Abuse, Alcohol or Opiate Withdrawal, Hypertension, Diabetes, Geriatric high fall risk, pain management, and wound care.
  • Unit Details: Use of electronic patient rounding and Rover system for documentation (pictures, vital signs, food intake).
  • Special Equipment: ObservSmart.
  • Shifts: Variable shifts including Day (7:00 AM - 3:30 PM), Evening (3:00 PM - 11:30 PM), and Night (11:00 PM - 7:30 AM).
  • Hours: 8 hours per shift, 40 hours per week.
  • Orientation Dates: May 30–31; shifts begin June 1.
  • Contract Length: 13 weeks.

Required Qualifications

  • Valid LPN / LVN license in California.
  • Minimum of 1 year of Long Term Care experience, with such experience obtained within the last year.
  • Behavioral Health experience.
  • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
  • Certification in Crisis Prevention Institute (CPI), ProAct, or PMAB (Management of Assaultive Behavior).
  • Management of Assaultive Behavior (MAB) certification.

Background and Compliance Requirements

  • 7-year background check including SSN trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening.
  • EPLS/GSA/SAM, HHS/OIG, OFAC Sanctions List, and Medi-Cal Suspended and Ineligible Provider List checks as applicable.
  • Education verification required unless highest education is high school.
  • Employment application must include full 7 years of work history with no gaps.
  • Employment verification for up to 7 years required; gaps over 30 days must be explained.
  • References: 2 professional references required for clinical candidates within 3 years of hire.

Health and Immunization Requirements

  • Physical exam within 1 year of start, including clearance for work.
  • Fit mask test within 1 year, renewed annually (specific mask types required).
  • Immunizations and/or titers required:
    • Tetanus/Diphtheria/Tdap (within 10 years).
    • Negative TB test or chest X-ray within 90 days, renewed annually.
    • Measles (Rubeola), Mumps, Rubella: Positive titers or vaccination series with boosters as needed.
    • Hepatitis B: Positive titer, completed series, or signed declination.
    • Varicella: Positive titer or vaccination series with boosters as needed.
    • Seasonal Flu vaccination required during flu season (Nov 1 - Apr 30) or signed declination.
  • Drug screening: Negative 9-panel drug screen within 6 months (excluding marijuana/cannabinoids per CA law).
  • Color blindness exam if applicable (in-person exam required).
  • Respiratory safety clearance required prior to start, renewed annually.

Competency and Testing

  • Medication Test/Pharmacology Competency Exam as applicable.
  • Unit-specific skills checklist required, renewed annually.
  • Dysrhythmia/EKG competency exam may be required for some telemetry units.
  • Additional competency exams as required by position.

Facility-Specific Documents and Compliance

  • Candidate must review and sign facility-specific documents prior to start, including:
    • Job description.
    • Standards of Business Conduct.
    • Code of Conduct.
    • Overpayment Refund Policy.
    • General Regulatory Orientation for Contracted Workers.
    • Vendor Training (2024 and 2025 versions depending on contract date).
