OT Home Health - Travel/Contract

Location: Marshfield, Wisconsin
Contract Length: 13 weeks

Job Details

• Specialty: Home Health Occupational Therapy
• Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
• Guaranteed Hours: 40 hours per week
• Average Patients per Day: 5-7 (to be confirmed during interview)
• Types of Cases: Geriatric
• Territory: Up to a 60-mile radius from the office; flexibility with territory expected
• On-call: Required every 3rd to 4th weekend
• Dress Code: Scrubs (any color), no jeans
• Equipment Provided: iPad
• Cell Phone: Not allowed for work use
• Mileage: Reimbursed at IRS standard rate
• Locals accepted at travel rate
• First-time travelers accepted

Required Qualifications

• Valid Occupational Therapist license in Wisconsin
• Minimum 1 year of Occupational Therapy experience within the last year
• BLS (Basic Life Support) certification

Background and Compliance Requirements

• Background checks must be completed within 90 days of start and updated annually, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records
  • Education and Work History Verification
  • State-specific searches as applicable (e.g., Kentucky, Virginia, Indiana, New Hampshire, South Carolina)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting may be required depending on state placement; must be current within 12 months
• Education verification of highest degree related to profession required
• Employment history verification for 7 years required with no gaps
• Two professional references required within 2 years of start (requirements vary by facility type)
• Signed Orientation Safety Manual Acknowledgement (updated annually)
• Agency Background Attestation required prior to start

Health and Immunization Requirements

• Physical exam within the last 12 months (not required annually)
• Negative TB test or chest X-ray within required timeframes; annual TB questionnaire required
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Flu seasonal vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required
• 9-panel drug screen within 12 months of start (instant acceptable)

Additional Facility-Specific Requirements

• Proof of current auto insurance required due to driving responsibilities
• Completion of Acceptable Use Policy prior to start
• Facility-specific forms may be required for certain states (e.g., Missouri Crime Disclosure, Flu and COVID statements)

Notes

• Flexibility with travel territory and weekend on-call is mandatory
• Timely and accurate documentation is critical for success in this role
• Scrubs dress code strictly enforced; no jeans allowed
• Mileage reimbursement provided for travel within territory radius
